#41: Tear Down that Wall! The Sheep and Shepherd Model
In the old days, everything was on the doctor, and in those days, it was mostly physicians. Doctors did house calls, in-office care, hospital care, and everything else. Those doctors would probably look at today's healthcare system with awe and amazement and a decent amount of head-spinning. That's because our understanding of the human brain and body and our focus on prevention has expanded greatly. There is now so much more to do for any given patient that it's nearly impossible for one person to take care of it alone. Any healthcare worker knows this just by looking at their overflowing inbox. The obvious solution to this conundrum is team-based care. But, the term team-based care is thrown around so flippantly in healthcare that it has lost much of its meaning. Building strong patient care teams is a critical step in preparing for the future of healthcare, decreasing burnout, and improving patient outcomes.  The Sheep and Shepherd Model turns the hierarchy intrinsic to healthcare on its head and systematizes the team-based care concept. But if it were that easy....everyone would already be doing it.
