Cervical checks during labor, while commonly practiced, can often be detrimental to the natural birthing process. These checks involve manually assessing the dilation of the cervix to determine the progress of labor. While the intention behind this practice is to provide a measure of how far along a laboring person is, it can inadvertently disrupt the physiological process.Shifting Focus from Sensations to NumbersA key issue with cervical checks is that they shift the laboring person's focus from their internal sensations to an external measurement. Labor is an intensely personal and inward journey, where the birthing person must connect deeply with their body’s signals. When a cervical check is performed, it interrupts this inward focus, making the laboring person concentrate on a numerical dilation measurement instead.This shift can create anxiety or disappointment, especially if the number does not align with their expectations or perceived progress. Such emotional responses can hinder the natural release of hormones like oxytocin, which is crucial for labor progression. Trusting one's body and its signals becomes more challenging when external validation seems to take precedence.Rapid Changes in DilationCervical dilation is not a linear process. A laboring person can go from one centimeter to full dilation (ten centimeters) in a remarkably short time—sometimes less than an hour. This rapid progression highlights the futility of relying on periodic checks to gauge labor's progress accurately. A seemingly slow dilation at one moment can quickly accelerate, making previous measurements obsolete and potentially misleading.The Case for Sensation-Based LaborFocusing on sensations rather than numbers allows the laboring person to stay attuned to their body's unique rhythm. Each contraction, each breath, and each movement plays a role in the birth process. By remaining inwardly focused, the laboring person can better respond to their body's cues, facilitating a smoother and more intuitive labor experience.Alternatives to Cervical ChecksInstead of routine cervical checks, birthing professionals can support laboring individuals by observing external signs of labor progression. These include changes in behavior, vocalizations, and the physical position of the baby. Encouraging a laboring person to trust their body and providing a calm, supportive environment can often be more beneficial than relying on numerical dilation measurements.Recognizing the rapid and unpredictable nature of cervical dilation further underscores the need to prioritize a laboring person's experience and intuition over routine checks. Dr. Sara Edwards has devoted her 35-year career to transforming nurse-midwiferyresearch, education, and clinical practice caring for women before, during and after pregnancy and birth. She is also a nurse researcher examining how the mother’s gut microbiome health impacts fetal and maternal health. Over her career, caring for tens of thousands of women and delivering 3000 babies, she noticed that women have frequent issues which she also experienced during her two pregnancies. Issues such as poor sleep, exhaustion, brain fog, rapid weight gain, poor digestion, heightened stress at work and at home, often lead to or coexist with anxiety and depressed mood. She sought to understand this all better, so she began her pioneering research of the maternal brain-gut axis and gestational weight gain in the perinatal population most atrisk, Black women and their infants. This NIH-funded and published research has the power to improve the lives of millions, particularly Black women, who are 3.5 times morelikely to suffer perinatal complications and death compared to U.S. White women.Throughout 20 years as a clinical professor at Emory University, her work extended to research protocols, educational programs, and multidisciplinary practices worldwide, enhancing the global visibility of nursing science. Her extensive clinical leadership in various practice settings, including as a founding midwife of the Atlanta Birth Center and now in solo practice at Pregnancy and Wellness Maui, together with her academic acumen have informed her work at the state, national and international levels. In Spring 2024, she launched a free preeclampsia screening app she co-designed called Materna BP, available for iOS and android phones. Today, Sara is CEO and founder of Sara Edwards Wellness, an online perinatal health coaching business for women wanting evidence-based support and guidance for an empowered pregnancy, strong baby, and a transformative labor and birth. What she has learned is that the fundamentals are key to robust health and these truths are largely ignored, if not undermined, by healthcare practitioners today. Many new midwives are nurses who left the hospital and many women who saw the rise in home birth and decided to join that profession. The shift of women desiring to birth at home is incredible, but I am finding that midwives are burning out and acting more like hospital protocols at home. THIS NEEDS TO STOP. I am a doula who has witnessed a plethora of births both completely undisturbed at home and induction at hospital. I come from a place of seeing the hospital births being scarier and less natural. The lies are unnecessary and unneeded. They happen to coerce women into speeding things up and rushing the process because hospitals have zero patience with birth. This is my observation. I see better birth outcomes at home. That is fact. The most recent home birth transfer to a hospital had me baffled by the amount of LIES I heard. This was into midwifery care in the hospital as well. Some that I heard were...-"The wireless monitor doesn't pick up the contractions as well as the wired one" (these monitors are not an accurate device at all at measuring if a mother is having contractions/how strong and I see it being used as a gas lighting tool far too often to push intervention) -"The baby's accelerations with the contraction is a sign that the baby is OP" (sunny side up)-"Pitocin does not stress the baby out and is the exact same thing as Oxytocin"-"A baby NEEDS to be stressed out before being born to help open their lungs" (this one had me fuming)-"With Preeclampsia the blood pressure can go up and down even if it stays down you still have it" (after I hydrated mom and her BP never once shot over 120/70 for the rest of birth 30+ hours) -"Contractions slowing down is bad"I am still upset that many women across the world experience these lies as well as many other lies to manipulate them into complying. Lately my 1 on 1 trauma birth story processing sessions have not been with hospital birth... but home birth or birth center births where the MIDWIFE was acting like and Obstetrician. This is almost worse. With a hospital you are at least aware of what you are getting yourself into. Protocols, machines, medicine, schedules, timelines, pressure to perform on time, giving birth with a surgeon. When a mom hires a midwife they are looking for someone to support their true physiological birth and no intervention. Almost every mother I talk to feels like they were lied to. "I'm a very hands off midwife, I fully support your natural birth." Then 39/40/41 weeks approaches and they are telling moms natural ways to induce labor. This goes back to episode 27 Why are we messing with God's design. Then followed by Yovana's incredible HBAC at 43.1. I had a friend go to a midwife's class for couples to learn how to "support during childbirth" and she came to me after and said "I almost laughed because she said "does everyone remember the midwife's trick?" and they all responded with "take benedryl!". When she told me this my blood boiled. Why are midwives teaching THIS versus how normal prodromal labor can be and how to truly navigate it. A woman will usually be able to sleep when the waves slow down (usually during the day).  If she trusts that what is happening to her body is normal and flows with it, it may even allow her to progress. Position of the baby can be addressed with some spinning babies or rebozo work, some massage and music by the partner or doula, and telling her to trust her body and rest between the waves is the BEST thing to do. Instead now it's benedryl, castor oil, evening primrose oil, blue and black cohosh.... why? This is only ADDING RISK to your birth! Midwives should know this! Do they not SEE the transfer rate when they add benedryl and just tell them to sleep? That doesn't help. Then when they cervical check after cervical check and the mom is swollen or doubting herself now she transfers.... the cascade begins with ANY intervention. Even the natural ones. Stop worrying about going past 42 weeks either! Maybe your body gestates longer!
